• At Pediatric Pain and Behavioral Health, services are provided “out of network.” This program does not participate in any insurance network because we feel strongly that third parties should not dictate the nature and duration of therapy. Additionally, instead of time communicating with insurance companies, this allows Dr. Foxen-Craft to focus on bringing the most value and highest standard of care. This includes cutting-edge evidence based practice, consultation with school and other providers, and communication between sessions.

You may also be eligible to seek insurance reimbursement for services provided at Pediatric Pain and Behavioral Health. To learn more about whether you are eligible, please contact your insurance company to determine your out-of-network mental health benefits. Each month (or upon request, more frequently), you will receive a Superbill, a specialized monthly itemized statement to submit to your insurance company.

If you call your insurance plan, we recommend asking the following questions:

Do I have mental health benefits?

Does my plan have out-of-network benefits?

What does the company allow (“reasonable and customary”) for the following CPT codes:

    90791

    90832

    90834

    90837

    90846

    90847

How much does my plan cover for an out-of-network mental health provider?

Are there standardized forms I will need to submit for reimbursement? Where do I find them?

How many sessions per calendar year does my plan cover?

    Are telehealth sessions covered?

  • Using a Health Savings Account (HSA) or Flex Spending Account (FSA) is an additional way to manage costs.

Brief Therapy Option: It takes time, dedication, hard work, and often financial resources to get care for your child and help them learn to manage their pain and behavioral health. That can be really difficult for many individuals and families who have already been struggling with their pain for a while. To help get you moving toward your goals as quickly as possible, and in an effort to help families manage their costs, Pediatric Pain and Behavioral Health offers a novel approach to therapy called the Fast Track to Self-Management. In this approach, core skills are introduced in the beginning of treatment. An estimated number of sessions based on your unique needs can be determined at the initial evaluation session (sometimes as few as 4-6). This may help your child gain skills effectively and efficiently, and can continue on a journey of self-management. Progress toward treatment is reviewed, and you and your family can work together to determine if you are interested in further treatment to continue to work on your goals.

  • If an appointment is cancelled with less than 48 hours notice, you will be charged he full cost of the session.

Cancellation fees cannot be submitted to insurance for out-of-network reimbursement.
